Documents, Forms, and Office Policies
There are several forms that you may read, print out, complete, and bring with you to your first appointment. Answering important questions before our appointment will allow us to start our session right away. If you are unable to read and complete the necessary forms before we meet, they will be provided to you to fill out at the time of your appointment.
You will receive a password to access additional forms when you schedule an appointment.
